Why keep a history?
From the 6/9 standup: the admin follow-up answers (GAP conversations, training agreements, knowledge assessments, agreed next steps and dates) are most useful as an ongoing record rather than something overwritten each year.
So each review writes a new dated entry. The current profile always reflects the latest, but the timeline preserves what was agreed and when — useful if a producer rebrands or details change between reviews.
Entries also capture who touched the profile — including views with no changes — and distinguish the admin who entered a note from the reviewer who had the field touch-point, so a shared team can see who's looked at what.

These entries are illustrative. The point is to confirm the shape of the record: date, reviewer, status, what was agreed, next follow-up date.
